Passenger Killed Friday Night in Suspected DUI Crash in Long Beach

wardlowatlantic.jpgLONG BEACH - 22-year-old Dana Acosta of Anaheim died following a suspected DUI accident late Friday night in Long Beach, the Long Beach Post reported. The incident was reported at the intersection of Atlantic Avenue and Wardlow Road at about 11:44 p.m. on Friday, April 1st. Heriberto Cervantes, 29, of Los Angeles, was driving a 1991 Toyota Camry north on Atlantic Avenue without his headlights on when he ran a red light at Wardlow. He swerved to avoid colliding with a 2005 Toyota Scion that was crossing the intersection on a green light. The Camry spun out of control and crash into a palm tree. Cervantes suffered multiple serious injuries to his upper body and was hospitalized. His passenger, Dana Acosta, died of her injuries shortly after arrival at the hospital. Cervantes also arrested on suspicion of felony drunk driving, driving on a suspended license, and gross vehicular manslaughter.

Two People Injured in Suspected DUI Head-on Collision in Phelan

The San Bernardino County accident was reported at about 8 p.m. on Phelan Road, west of Monte Vista Road. According to reports, Suzy Ko, 43, of Victorville, was travelling east on Phelan Road when she allowed her vehicle to drift into oncoming traffic. She struck another vehicle driven by a 24-year-old woman. The 24-year-old woman and her 15-year-old passenger were both seriously injured and had to be flown to Arrowhead Regional Medical Center. Suzy Ko was arrested and booked in the Victor Valley County Jail.

Cause of the Phelan Road Crash

The California Highway Patrol is the agency investigating this head-on collision. If news accounts of the March 10th incident are accurate, Suzy Ko is likely responsible for this collision, because she allowed her vehicle to drift into westbound traffic and strike another vehicle head-on. Two Killed by Wrong-Way Driver on 91 Freeway in Anaheim

CA-91.pngANAHEIM -- A 26-year-old Diamond Bar man and a 24-year-old Long Beach woman were killed in a wrong-way collision early Wednesday morning on the 91 Freeway in Anaheim, the LA Times and multiple other news outlets reported. According to news accounts, 21-year-old Anthony Louis Fragoso, 21, of South Gate, was driving west in the eastbound 91 carpool lane at about 12:50 a.m.

As Fragoso approached the off-ramp at Harbor Boulevard, he collided head on with at 2006 Toyota Scion. The driver of the Scion, Alan Reyes, 26, was ejected onto the freeway.

Both Reyes and his passenger, Vanessa Cease, 24, of Long Beach, died at the scene of the crash.

Fragoso suffered major injuries in the collision and was taken to UCI Medical Center in Orange. He was arrested at the hospital charges of vehicular manslaughter with gross negligence. All lanes of the freeway were closed for about three hours while the California Highway Patrol Investigated.

Jury Awards $14 Million to Family of Two Boys Killed in DUI Accident

The drunk driver and only defendant, George Butler, 36, was driving at speeds of about 80 mph when he hit the truck carrying Jordy Martinez, 1, and Jesus Martinez, 6, on January 14, 2007. In 2008, Butler was sentenced to 15 years in prison, followed by years of probation and community service. The award consisted of $12 million for pain and suffering of the parents and surviving brother, and the remainder of the damages went to past and future medical bills and funeral expenses. The jury awarded $25,000 in punitive damages.

Huntington Park Man Killed in Downey Motorcycle Accident; Driver Arrested

paramount-florence.jpgA 23-year-old Huntington Park man riding a motorcycle was tragically killed early Friday morning when his motorcycle collided with a pickup truck that ran a red light in Downey, the Long Beach Press Telegram reported. The accident occurred around 2 a.m. at the intersection of Paramount Boulevard and Florence Avenue, Police reported. The driver of the 2001 GMC pickup truck, a 31-year-old Los Angeles man, was arrested on suspicion of felony drunken driving. The pickup truck, travelling east on Florence Avenue, struck the motorcycle, which was traveling southbound on Paramount Boulevard. The name of the victim and the driver were not immediately released.

Two Men Killed, Three Injured in Suspected DUI Crash Near Terra Bella

drinking-driving-eb.jpgThe CHP has identified the two victims of a horrific double-fatal crash near Terra Bella Friday night as Rafel Mendez, 56, of Portville and Juan Reyes, 53, of unknown city of residence, according to the Porterville Recorder. Both Mendez and Reyes were passengers in a Toyota driven by Adrian Ramos, 23, of Mecca. According to reports, Ramos was driving a 2005 Toyota southbound on Road 208, while at the same time, Porterville resident Manuel Valencia, 69, was driving a Mercury westbound on Avenue 96. Ramos reportedly failed to stop at the stop sign at the intersection and collided with the Mercury.

Adrian Ramos, Manuel Valencia and a passenger in his vehicle, Juan Hernandez, 49, all suffered major injuries. Both Ramos and Valencia were taken to Community Regional Medical Center in Fresno. Hernandez was taken to Sierra View District Hospital. Mendez and Reyes died as a result of their injuries. Adrian Ramos was arrested on suspicion of drunken driving.

Five Injured in Anaheim Crash on Interstate 5; Oceanside Man Arrested On Suspicion of DUI

According to the OC Register, the suspected alcohol-related accident occurred on northbound Interstate 5, south of the 91 freeway, around 12:26 a.m., according to the CHP. The suspected drunken driver, Sabah Mikha, was driving a 2004 Mazda that struck a 1993 Ford Explorer and caused it to overturn. The five occupants of the Explorer, Maria Garcia-Salas, 24, the driver, and passengers Elizabeth Garcia, 22, Rosa Garcia, 5, Jesus Garcia, 20, and Evelyn Aguilar, 19 suffered injuries ranging from minor to moderate.

73-Year-Old Solvang Woman Killed, Three Injured in Highway 154 Accident

A 73-year-old Solvang woman tragically died Wednesday night in a violent multi-vehicle accident on Highway 154 (San Marcos Pass Road) near Lake Cachuma. Authorities believe alcohol played a role in the September 15th crash, which involved five vehicles.

Around 8 p.m. Wednesday, Priscilla A. Susman, 54, was seen driving a 1996 Nissan Pathfinder erratically on Highway 54 toward the Santa Ynez Valley, near Lake Cachuma. Witnesses reported she made an illegal U-turn and began heading back toward Santa Barbara.
highway-154-santa-barbara.jpgAccording to news reports, Susman's vehicle was seen weaving over double yellow lines and into oncoming traffic. Susman then sideswiped a 2006 Pontiac driven by Syrena M. Hernandez, 24, of Oxnard. Both vehicles spun out of control. Fortunately, Hernandez was not hurt.

Susman's Pathfinder, which ended up in oncoming traffic, was then struck by the 73-year-old Solvang woman driving a 1989 Saab, and then a 2005 Toyota Prius, driven by Linda J. Gaytan, 53, of Santa Ynez. A 2007 BMW, driven by Jay B. Fortman, 55, of Santa Barbara, reportedly struck Hernandez's Pontiac.

Susman suffered major chest trauma and was taken to Santa Barbara Cottage Hospital. Despite being in critical condition, she was arrested on charges of driving under the influence and gross vehicular manslaughter while intoxicated.

The 73-year-old Solvang woman, who was not identified, died in the crash. Gaytan and Fortman both suffered blunt force trauma injuries and were taken to Santa Barbara Cottage Hospital.

Highway 74 Rollover Accident Seriously Injures Passenger

ca-74.jpgA 19-year-old woman suffered major injuries late Wednesday night when the vehicle she was riding in overturned on a curve on Highway 74, the Desert Sun reported. The CHP received reports of the solo vehicle accident at 11:38 p.m. Wednesday, September 15, 2010, on Highway 74 between Vista Point and Palowet Drive. The driver of the vehicle, a 22-year-old man, suffered non-life-threatening injuries and was taken to Desert Regional Medical Center on Palm Springs. He was later booked into the Riverside County Jail in Indio on suspicion of felony DUI. The front-seat passenger of this vehicle, a 19-year-old woman, suffered major injuries and was also taken to Desert Regional.

south-harbor.jpgPolice yesterday arrested 19-year-old Beau Richard Cook, six days after he struck and killed a pedestrian in Santa Ana, the Orange County Register reported. The fatal accident occurred last Wednesday, just before 11:30 p.m. According to news reports, Cook was speeding southbound on Harbor Boulevard in a 2010 Scion when he may have run a red light at Warner Avenue. He lost control of the car, struck a curb and knocked over a fire hydrant. Eloise Aguilar of Santa Ana, who was walking home from work, was struck by Cook's Scion and was killed instantly. Cook was taken to the hospital with injuries to his pelvis and left leg. He was arrested Tuesday shortly after being released from the hospital. He is facing felony charges of vehicular manslaughter along with an enhancement for driving under the influence of alcohol.

Mother and Two Children Struck by Tow Truck Driver in Oxnard Crosswalk

simon-vineyard.jpgA tow truck driver was arrested Tuesday afternoon after striking a mom and her two young children in a marked crosswalk in Oxnard. The accident occurred at the intersection of Vineyard Boulevard and Simon Way. According to police, the tow truck driver was turning left from westbound Simon Way to southbound Vineyard Avenue on a green light. The young mother, Alma Lopez, 19, was pushing her stroller across Vineyard on a green light in a marked crosswalk. Lopez was struck by the tow truck, driven by Mehran Khamseh, 45, of Santa Barbara, and thrown to the ground, and the stroller was pinned under the flatbed truck and dragged more than a quarter of a mile. The accident occurred at about 12:07 p.m. Tuesday.

Lopez suffered moderate injuries and her two children, a 3-year-old girl and a 1-year-old boy, suffered critical injuries. They were treated at Ventura County Medical Center. Khamseh was later arrested on suspicion of driving under the influence of drugs. According to the Ventura County Star, Khamseh was held in jail without bail on suspicion of DUI, parole hold and immigration hold. Court records showed several prior traffic infractions. Khamseh was also convicted in 2008 of making terrorist threats and petty theft with a prior, both felonies.

The Oxnard Police Department is still continuing their investigation. Diamond Bar Woman Killed in Suspected DUI Crash in Anaheim

ca-57.pngA 22-year-old Diamond Bar woman tragically was killed Sunday night in a likely alcohol-related crash in Anaheim, the Orange County Sheriff-Coroner reported. The accident occurred on the southbound 57 Freeway (Orange Freeway), near the 91 Freeway (Riverside Freeway), at about 11:15 p.m. on Sunday, August 22, 2010. The victim, Mai Hayakawa, 22, was a passenger in a 2003 Toyota Celica driven by Michael Keating, 24, of Fullerton.

Keating, who was later arrested on suspicion of driving under the influence of alcohol, was attempting to merge onto westbound 91. He reportedly sped up to pass a big-rig on the shoulder, and lost control of his vehicle. The Celica veered down an embankment and struck a palm tree. Hawakawa died at the scene of the crash. Keating and a 22-year-old woman in the backseat of the vehicle suffered minor injuries and were taken to UCI Medical Center.

The cause of the fatal 57 Freeway crash remains under investigation by the CHP. However, if news reports are accurate, this fatal solo-vehicle Orange County crash was caused by the suspected drunk driver of the vehicle. The solo accident occurred on northbound Bouquet Canyon Road, just north of Texas Canyon Road, shortly after 2 a.m.

The driver of the 2001 Porsche, 24-year-old Michael Hemperly Jr. of Palmdale, lost control of his vehicle at an unknown speed and for unknown reasons. The passenger side of the vehicle struck a tree, and the passenger, 26-year-old Marissa Hodges of Rancho Cucamonga, who was reportedly not restrained, died at the scene of the crash.

Hemperly, who was wearing a seatbelt, suffered only minor injuries and was taken to Henry Mayo Newhall Memorial Hospital. He was arrested on suspicion of driving under the influence causing great bodily injury and booked at the Santa Clarita Valley Sheriff's Station at about 5 a.m., where he was held on $130,000 bail.

The CHP is investigating the cause of the accident and are investigating the speed of the vehicle and all possible contributing factors. Suspected Drunk Driver Arrested after Passenger Dies in Ventura County Crash

A 22-year-old Carpinteria woman died early Sunday morning after an early morning crash caused by a suspected drunk driver, the Ventura County Star reported. The victim, Elizabeth Gonzalez, was one of two passengers in a vehicle driven by Alma Ruiz, 29, of Santa Barbara. Around 2 a.m., Ruiz was driving eastbound on Bates Ranch Road, near Highway 101 and Rincon Beach Park, when she apparently lost control of her vehicle on a curve and struck a power pole. The pole was sheared off at its base and fell into the road. The car directly behind Ruiz's car also crashed into the wreckage. Ruiz and the other passenger in her vehicle, Yahaira Martinez, 21, of Carpentaria, both suffered moderate injuries. Gonzalez died of her injuries at the scene of the accident.

Driver Facing Vehicular Manslaughter Charges After Rancho Cucamonga Man Killed in Head-On Collision

A Rancho Cucamonga resident died Sunday night after a head-on collision on Church Street in Rancho Cucamonga, police reported. The accident occurred at about 9:13 p.m. on Church Street, near the intersection of Elm Avenue. According to the San Bernardino County Sheriff-Coroner Department, the victim, Paul Martin, 64, of Rancho Cucamonga, was driving east in his 2007 Hummer H3 on Church Street near Elm Avenue. At the same time Albert Ruiz, 23, of Rancho Cucamonga, was driving his 2005 Nissan Sentra westbound on Church Street.

Ruiz's Nissan reportedly struck the north curb and then crossed the center median and into westbound traffic. The front of the Sentra collided with the driver's door of Martin's Hummer. Both Martin and Ruiz were transported to local hospitals. Martin died of his injuries at San Antonio Community Hospital. Ruiz was treated for minor injuries at Arrowhead Regional Medical Center in Colton and later booked on charges of Vehicular Manslaughter, PC 192(C)(1) at the West Valley Detention Center.

Cause of the Crash

The Rancho Cucamonga Major Accident Investigations Team is investigating the cause of the June 13th accident. According to the San Bernardino Sun, blood tests taken at the hospital showed Albert Ruiz's blood-alcohol level was twice the legal limit. Ruiz is facing criminal charges for this accident and the resulting death of Paul Martin.
